Village Health Navigator
Cross Keys Village – The Brethren Home Community (CKV) is seeking a compassionate and service‑oriented professional to join our team as the Village Health Navigator, helping us support the well-being of our residents in a faith‑based, collaborative environment.
This opening is for a full-time salaried position.

Job Description
As the Village Health Navigator your scope of responsibilities will include but are not limited to:
- Establishing, maintaining and evaluating programs to meet department objectives and the long-range strategic goals for the Village Health Services Department as related to the operations of providing psychosocial support to the Villagers. Collaborating with the Director of Village Health Services to develop and implement policies, procedures and programs for Village Heath Services Navigators.
- Assessing Villager’s bio-psycho-social needs and assists them to access resources/support to optimize their well-being and maintain their highest level of functioning and in their preferred location.
- Exploring and evaluating Villagers coping mechanisms and assists with teaching effective therapeutic techniques/exercises to optimize their quality of life.
- AssistsingVillagers, families and the care team with the transitions across the continuum of care by conducting interviews, assessments and documentation in compliance with the CKV-TBHC’s policies and State/Federal regulations. This may require hospital, home, or other continuum of care visits.
- Assisting with adjustment through the admission, residency and/or discharge/death/transfer phases. Entering a therapeutic alliance with the Villager as he/she strives to retain his/her identity and maintain his/her autonomy while trying to optimize psychosocial functioning.
- Providing education to Villagers and families on the aging and disease processes, as well as neurocognitive disorders, behavioral health disorders and other related disorders that impair or impact their daily functioning.
- Maintaining an up-to-date knowledge of Federal and State regulations pertaining to Social Services. Completes required documentation and record keeping. Performs mood, cognition, behavior, and needs assessments.
- Collecting Villager information to help other health professionals understand the emotional, social, spiritual and environmental factors that may impact his/her well-being. Acting as an advocate, intervening on behalf of Villagers with the community, team members, professionals and community providers of service. Maintains professional relationship with community resources.
- Completing treatment and community referrals as indicated.
- Actively serving as a member of the Residential Living Care Team and providing leadership in representing the clinical social work perspective/liaison with the Villager and family.
- Working closely with Residential Living, Personal Care and Brookside Teams as well as the Healthcare Center Admissions Department and Memory Support to determine the service area that best supports each Villager’s needs all while promoting the highest level of independence possible.
- Assisting Villagers and families with emotional, psychological and spiritual issues. Coordinates services with other Care Team Members.
- Collaborating closely with the Villager, family and community team members to create and implement transition plan. As Villagers transition to a higher level of care, providing support before, during and after the transition.
- Assisting with planning and presenting Social Service and Memory Support programs, as assigned. When requested, attending functions which promote the mission of CKV-TBHC.
- Performing various administrative duties including telephone calls, maintaining Villager’s records and completing various forms as requested.
Qualifications, Education and Experience
- Must possess a Bachelor’s Degree in social work, human service, or a related field.
- One to three years of social work or human service experience in a health care setting is preferred.
- Must possess a working knowledge of dementia and mental disorders and understand both psychosocial and pharmacological approaches to the treatment of emotional and behavioral disorders encountered most often in the long-term care setting.
- CPR certification is preferred.
