Cross Keys Village - The Brethren Home Community (CKV) is currently seeking Licensed Practical Nurses to become an integral leader on our PC team and help provide our residents with the highest quality of care. Personal Care, more commonly referred to as PC Mission Point, is designed for individuals who wish to remain independent when staying at home has become impractical or unsafe. Job Description

As a Charge LPN, your scope of responsibilities will include but is not limited to:

• Identifying, coordinating, implementing, evaluating and monitoring health services for Personal Care (PC) residents.
• Providing guidance and leadership to PC team members and participating in performance evaluations.
• Participating in the resident admission process to ensure tasks are completed timely and appropriate support plans are implemented.
• Identifying and performing in-depth assessments of resident’s physical status and referring residents to a physician or appropriate agency as medically appropriate.
• Completing required documentation and record keeping accurately and timely in an electronic care tracker.
• Assisting physicians on resident appointments, following physician orders and ordering medications as prescribed.

Education and Experience

• Must be licensed as a practical nurse in the State of Pennsylvania.
• Post-licensure experience preferred but not required.
• Must demonstrate CPR competencies.
• Evidence of continuing education in line with community, state and federal regulations.
• Must possess quality mindedness, critical thinking, initiative in leadership responsibilities, interpersonal sensitivity and communication skills.
