Cross Keys Village - The Brethren Home Community (CKV) is currently seeking a Dementia Care Coordinator to provide leadership to our team and help ensure our residents are being met with the highest quality of care. This opening is a full-time, day shift position with an on call weekend and holiday rotation.

In our Health Care Center, more commonly referred to as Mission Point, you will find our outstanding nursing team assisting residents in five distinct neighborhoods – specializing in long-term care, memory care and short-term rehab.


Cross Keys Village - The Brethren Home Community is currently seeking a Dementia Care Coordinator to join our memory care neighborhood, know as Somerset. Somerset is a specialized environment for individuals with a diagnosis of neurocognitive disorder or dementia, designed to provide an optimal environment during the late stages of their disease. CKV team members work together to provide Somerset residents with guidance, cueing, engagement, socialization and physical care to support their best life.
 

Job Description

As a Dementia Care Coordinator, your scope of responsibilities will include but is not limited to:
 

  • Overseeing the life of the Neighborhood. Utilizing a leadership style which promotes collaboration, coaching, and teamwork. 
  • Providing guidance for the clinical and non-clinical team on the neighborhood for the 24 hour day. Flexing hours across shifts based on needs of the facility.
  • Ensuing the neighborhood operations are in compliance to policies and State/Federal licensing regulations and also with the needs of the residents’ and plan of care.
  • Assisting in licensure surveys.
  • Resolving any concerns involving the team, residents, and families. Actively participate in meetings such as IDT’s, Neighborhood Meetings and rounds for the neighborhood.
  • Responsible for interviewing and hiring neighborhood team members in conjunction with the team and Human Resource Policies.  
  • Coordinating and/or providing education for the team on topics such as risk management, customer service, and mission of the neighborhood, function and flow of the neighborhood.
  • Assisting the Interdisciplinary Team in monitoring of clinical areas, Resident Rights, abuse prevention and behavioral management. Working closely with the Interdisciplinary Team to provide maximum quality of life and quality of care.
  • Providing assistance in coordination of admissions to and discharges from the neighborhood which includes welcoming and orienting the resident and their family.
  • Promote an environment of the neighborhood which is reflective of home, clean, safe and visually appealing.
  • Collaborating extensively with the NHA, Director of Memory Support, and DON. Complete reports as needed to monitor the success of the neighborhood.
  • Demonstrating the ability to empower through active and visible leadership, collaboration with other neighborhoods and represent neighborhood at key community functions and meetings.
  • Assisting in completing any required documentation and record keeping to maintain a complete, accurate medical record. Reviews daily clinical nursing report to identify status changes. 
 

Education and Experience

  • Minimum 5 years of experience in dementia care, Leadership experience required.
  • Bachelor’s Degree in a Human Services field preferred (i.e. Human Development and Family Studies, Special Education, Rehabilitation Counseling or Psychology) or related skilled long-term care experience.
  • Ability to communicate clearly verbally and in writing required.
