Cross Keys Village - The Brethren Home Community is searching for an experienced Registered Nurse to fulfill the role of the Director of Personal Care Clinical Services. This unique position enables an RN with the opportunity to combine her/his experience as a skilled clinician, educator, and supervisor in a vibrant and growing community. 


This opening is for a full-time salaried position working Monday through Friday 8:00 a.m. - 4:30 p.m. Occasional evening, night and weekend hours may be required.

Mission Point Personal Care at Cross Keys Village, more commonly referred to as PC Mission Point is designed for individuals who wish to remain independent when staying at home has become impractical or unsafe. Some of our residents might have limited mobility whereas others might be experiencing various degrees of memory loss. Regardless of their circumstance, Personal Care at CKV actively promotes residents aging in place. Our team members work together to create a more active and purposeful life for our residents, so they can lead healthier and more fulfilled lives.

Brookside Memory Care Residence at Cross Keys village, is a specialized personal care environment for individuals with a diagnosis of neurocognitive disorder or dementia, designed to provide an optimal environment during the intermediate stages of their disease. CKV team members work together to provide Brookside residents with guidance, cueing, the right level of stimulation and interaction with others who understand the progression of their condition.

 

Job Description:

As the Director of Personal Care Clinical Services, your scope of responsibilities will include but is not limited to:

  • Demonstrating clinical expertise in the field of geriatric health. Making decisions based on established best practices and standards of care.
  • Striving to ensure residents’ physical, psychological, social, intellectual, and spiritual well-being. Working collaboratively with other departments to meet residents’ needs. Holding clinical team appropriately accountable if needs are not being met.
  • Reviewing the 24-hour report and identifying clinical areas in need of attention.
  • Overseeing clinical team meetings and encourages team to engage in collaborative problem-solving strategies to provide the highest level of resident care. Sharing clinical expertise to make sound decisions regarding Resident care.
  • Conducting clinical rounds on all neighborhoods and shifts to engage with the team and monitor neighborhood performance. Addressing any concerns in a timely manner. Using rounding as an opportunity to connect with clinical team members and provide ongoing education.
  • Evaluating the outcome of the clinical process to ensure quality Resident care, positive team member relations, and efficiency of service.
  • Working collaboratively with Medical Director/s & Pharmacist Consultant/s. Maintaining ongoing communication with clinicians about Residents’ needs and regulatory compliance. Addressing any clinical and documentation concerns, as needed.
  • Teaching Medication Administration classes on a regular basis. Conducting medication observations as required by regulations.
  • Identifying clinical training and competency opportunities and arranges training to keep clinical skills of the teams at the highest levels.
  • Mentoring nurses on leadership team and identifies professional growth opportunities. Keeping own level of clinical knowledge and leadership skills up to date.
  • Demonstrating effective written and verbal communication skills in all interactions. Ensuring effective inter- and intra- departmental communication, and practices effective communication skills with internal and external customers.
  • Fostering teamwork and team member engagement among all staff. Serves as a role model of Leadership.
  • Playing an active role in Admissions to Personal Care and Brookside. Confers with Admissions Counselor and provides clinical assessment and input into admission decisions.
  • Facilitating communication with team members, residents and family members and ensures prompt responses to questions or concerns.
  • Fostering an interdisciplinary approach to enriching the lives of residents. Addressing concerns about resident care and protocols with other department leaders in a professional and timely manner
  • Promoting and supporting the goals and objectives of CKV-TBHC and ensures that the information is shared in a positive and effective manner with the clinical department.
  • Educating, directing, and evaluating nursing leaders; Providing timely feedback about performance. Helping direct reports to establish goals and encourages professional growth. Assisting in the development of leadership skills among Nursing team leaders.
  • Assisting the Personal Care Home and Brookside Administrators to prepare the annual budget. Demonstrating understanding of fiscal considerations in goal setting and decision-making.
  • Responsible for monitoring expenses for personnel, supplies, and equipment while conforming with budgetary guidelines.
  • Overseeing and ensuring the clinical team complies with CKV-TBHC’s policies and state regulations. Staying apprised of all regulatory policies and protocols and communicating updates to the clinical team in an effective, timely manner.
  • Establishing objectives, organizing, and managing the activities of the clinical teams to provide quality care for all residents in accordance with facility procedures and state regulations.
  • Ensuring adequate, efficient staffing patterns based on quantified measurements.
  • Actively participating in state surveys, submitting reportable events, and preparing plans of correction in a timely manner.
  • Identifying appropriate, efficient methods of collecting and implementing clinical data. Performing periodic evaluation of documentation methods.
  • Performing periodic evaluations of resident support plans.
  • Ensuring that residents’ rights are protected per the Resident’s Bill of Rights.
  • Fills in for the Personal Care Home or Brookside Administrators during times of absence.
  • May work as Charge Nurse in the event of an emergency.

Education and Experience:
 
  • Must be licensed as a Registered Nurse in the State of Pennsylvania.
  • Bachelor’s degree in nursing and minimum of five years of supervisory management experience in a long-term care setting is preferred.
