In our Health Care Center, more commonly referred to as Mission Point and licensed for 270 beds, you will find our outstanding nursing team assisting residents in five distinct neighborhoods – specializing in long-term care, memory care and short-term rehab. Our team members work together to create a more active and purposeful life for our residents, so they can lead healthier and more fulfilled lives.

Job Description:

As a LPN, your scope of responsibilities will include but is not limited to:

• Coordinating, planning, implementing and evaluating nursing care to maintain the resident’s highest practicable physical, mental and psychosocial wellbeing.
• Assessing residents for condition changes, administering medications and completing required documentation thoroughly and accurately in an electronic care tracker.
• Supervising and monitoring the delivery of care by CNAs according to resident care plans.
• Monitoring dining activities to identify concerns or changes in resident’s needs.
• Interacting with families and visitors and accompanying physicians on rounds.

Education and Experience:

• Must be licensed as a practical nurse in the State of Pennsylvania.
• Post-licensure experience preferred but not required.
• Must demonstrate CPR competencies.
• Evidence of continuing education in line with community, state and federal regulations.
• Must possess quality mindedness, critical thinking, initiative in leadership responsibilities, interpersonal sensitivity and communication skills.
