Cross Keys Village – The Brethren Home Community (CKV) is seeking a friendly, organized professional to support our memory care programs. As the Memory Support Assistant, you will partner with the Director of Memory Support to coordinate programs, events, education, outreach, and key administrative tasks that help our services run smoothly and meaningfully for participants, team members and care partners.


Position Hours: Full-time day shift with the flexibility to to work evening and weekend hours as needed.

Your Role as the Memory Support Assistant
 

As the Memory Support Assistant, your scope of responsibilities will include but are not limited to:

Program & Event Support

  • Setting up rooms for Memory Support programs and groups.
  • Preparing and managing program materials: sign-in sheets, name tags, table tents, evaluation forms, educational packets.
  • Managing supplies, order necessary items, and prepare packets for community outreach.
  • Documenting attendance and program participation.
  • Coordinating RSVPs, room reservations, charter bus arrangements, catering, and participant communications for events.
  • Engaging in communication with community partners.
  • Supporting social gatherings or weekend classes.

Referral & Documentation Support

  • Monitoring incoming Memory Care Coach referrals and maintaining referral logs.
  • Preparing documentation templates and ensuring consent forms are completed.
  • Maintaining secure records and assisting with post-intervention follow-up as needed.
  • Organizing participant information for support groups and tracking programs.

Education & Training Support

  • Learning and utilizing Relias for course management.
  • Uploading courses, PDFs, and training materials for Memory Support programs.
  • Tracking attendance, assembling participant folders, and preparing PowerPoint presentations.
  • Assisting with Symposium Day, specialty classes, and evaluation summaries.

Marketing & Communication Support

  • Drafting flyers, updating marketing content, and coordinating community outreach materials.
  • Contacting speakers for events and maintain a community contact database.
  • Tracking marketing deadlines, managing mailing lists, and assisting with press releases.

Music & Memory Support

  • Maintaining participant music preference forms and organizing devices.
  • Sanitizing, charging, and tracking iPod devices, pillow speakers, and headphones.
  • Demonstrating device usage to team members, residents, and families, and documenting feedback.

Participant & Care Partner Support

  • Making reminder calls, welcoming new participants, and completing demographics information.
  • Conducting check-in calls after support groups and provide basic resource information under guidance of Director.

Data & Outcome Tracking

  • Tracking attendance trends, compiling evaluation data, and preparing quarterly summaries.
  • Maintaining spreadsheets for outreach efforts and program impact measurement.

Special Projects

  • Confirming and reminding guest speakers for events.
  • Organizing therapeutic activity materials and researching community resources.
  • Assisting in preparing board or executive updates.
  • Maintaining lending library and overflow cabinets.
  • Becoming a Dementia Friend within first month of employment.
  • Conducting research on dementia and Alzheimer’s topics and managing memberships with relevant organizations.

Reporting & Escalation

  • Reporting participant or care partner concerns to the Director of Memory Support.
  • Notifying Director of cancellations or scheduling changes for programs, events, or educational sessions.

 

Required Skill Set
 

  • Must be compassionate, professional, and able to interact effectively with older adults and care partners.
  • Must be Proficient with Microsoft Office and be open to learning new database systems.
  • Ability to work collaboratively and support the vision of the Memory Support team is required. 
  • Strong Organizational and administrative skills are required.
