Cross Keys Village - The Brethren Home Community (CKV) is currently seeking a skilled and compassionate professional to assist in developing and implementing social service and memory care support for our villagers and residents living on campus. As a Resident Support Specialist working at CKV, we will provide you with opportunities to enjoy a friendly work environment guided by Christian values.

This opening is a full-time position working Monday - Friday 8:00 a.m. - 4:30 p.m. Evening and weekend on-call duties may be assigned.

Job Description:

As a Residential Support Specialist, your scope of responsibilities will include but is not limited to:

• Assessing resident’s bio-psycho-social needs and assisting them with accessing resources/support to optimize their well-being and maintain their highest level of functioning.
• Assisting with adjustment through the admission, residency and/or discharge/death/transition phases. Entering a therapeutic alliance with the resident as he/she strives to retain his/her identify and maintain his/her autonomy.
• Performing mood, cognition and behavioral assessments.
• Exploring and evaluating residents coping mechanisms and assisting with teaching effective therapeutic techniques/exercises to optimize their quality of life.
• Providing education and support to the resident, families, and team members regarding neurocognitive disorders, behavioral health disorders and other related disorders that impair or impact their daily functioning.
• Completing treatment and community referrals as indicated.
• Collaborating with other service lines across campus to determine the service area that best supports each resident’s needs while promoting the highest level of independence possible.
• Working closely with residents, families and community team members to create and implement a transition plan. Providing support before, during and after the transition process.
• Acting as the resident advocate, intervening on behalf of residents within the community, team members, professionals and community providers of service.
• Maintaining professional relationships with community resources.
• Facilitating therapeutic groups.
• Completing required documentation and record keeping.

Education and Experience:

• Bachelor’s degree in social work, human service, or a related field is required. 
• One to three years of social work or human service experience in a long-term care/community setting is preferred. Experience in Memory Care is highly desired. 
• Must possess a valid driver's license.
